Once we receive a referral, our team reviews the information provided and gets in touch to discuss the participant’s needs in more detail. This gives us the opportunity to understand their current situation, preferred supports and any health or care requirements that may need to be considered.
If Faith Care 4U is suitable, we can then discuss the next steps and how support may be arranged. We aim to keep communication clear throughout the process so participants, families and referrers know what is happening and what to expect.
